Back Pain is a SYMPTOM.
(Yes! you read right!) It is a indication that there is a problem, it is not the 'problem' itself.
Spasm or stiffness is a SYMPTOM
It's your body's way of splinting your spine in response to a deeper underlying CAUSE.
Radiating leg pain or numbness in the leg is a SYMPTOM-
of nerve irritation or compression due to a deeper underlying CAUSE..
A SYMPTOM is defined as an indication of an underlying cause or problem. And if you only intervene at the symptom level, you are not dealing with the real CAUSE of the problem- the reason behind your symptoms.
For eg. Cough is a symptom but has different causes behind it - allergy, acid reflux, lung infection, side effect of certain medication etc. so if you only treat the cough with chewable lozenges to soothe the throat or medicines that suppress the cough, and not treat the underlying cause, can you truly get better? Of course not! Your cough comes back, the underlying cause stays unresolved and gets worse over time.
Examples of common SYMPTOMATIC treatments for Back Pain include Passive interventions such as: painkillers, stretching muscles that feel 'tight', pain patches, muscle relaxants, massages, spinal adjustments/ manipulations, dry needling, acupuncture, etc
If you have tried Active interventions such as back or core strengthening exercises, yoga or Pilates and it isn't helping- it is mostly because they don't address your particular pain mechanism (the CAUSE) or it is unknowingly feeding into your pain mechanism!
Using generic and symptomatic treatments for a repeating case of back pain is like giving an Aspirin to a patient with a brain tumour. It may relieve the headaches but it isn't going to do a darn thing to the tumour.
